Process for Complaints by Outside Individuals

An individual who is not a USC student, faculty member, or staff member can make a complaint about the USC Division of Biokinesiology & Physical Therapy.

The complaint should be submitted in writing to the Division Chair (http://pt.usc.edu/about_the_division/contact_us/) and should include the following information:

Name and contact information of the person filing the complaint
Name of the person(s) about whom the complaint is filed
Details of the complaints, including dates and locations

Upon receipt of the complaint the Division Chair will either handle it directly or forward it to the appropriate University office. If not handling it directly, the Division Chair will communicate with the individual making the complaint about its status. If the Division Chair handles the complaint directly, he or she will send a letter summarizing the decision about the complaint to the person involved.

The aim of this process is to resolve all matters as quickly as possible. However, some issues may require more time to be fully investigated. If so, the person who initiated the complaint will receive an interim response describing what is being done to deal with the matter, and when a decision can be expected.

The Division of Biokinesiology & Physical Therapy maintains records of complaints in compliance with USC policies and procedures and applicable law. Information reported in this complaint process will be treated as confidential to the extent allowed by law; however, mandatory reporting may be legally required for certain types of complaints.
